Conclusion of contracts

Contractual relations between producers and consumers:

Contractual relations regarding the use of centralized municipal water supply and sewage systems are carried out exclusively on a contractual basis in accordance with the Laws of Ukraine "On Drinking Water and Drinking Water Supply", "On Housing and Communal Services", the Rules for the Use of Centralized Municipal Water Supply and Sewage Systems in Settlements of Ukraine, approved by the order of the Ministry of Housing and Communal Services of Ukraine dated June 27, 2008 No. 190, the "Rules for the Technical Operation of Water Supply and Sewage Systems", approved by the order of the State Housing and Communal Services of Ukraine dated July 5, 1995 No. 30; the "Rules for the Acceptance of Wastewater from Enterprises, Institutions, and Organizations into the Sewerage System of Odessa", approved by the decision of the Executive Committee of the Odessa City Council dated June 14, 2007 No. 632; technical conditions, existing GOST standards, and other regulatory acts adopted in accordance with the law.

Essential conditions of the contract between the producer and the consumer of centralized water supply and sewage services are determined in accordance with the Laws of Ukraine "On Drinking Water and Drinking Water Supply", "On Housing and Communal Services".

To conclude contracts for the provision of centralized water supply and sewage services, legal entities and individual entrepreneurs should contact the Control and Industrial Water Use Department of the "Infoxvodokanal" branch at the address: Odessa, 43a Akademika Korolyova Street, contact phone: 705-55-21.

To conclude contracts, owners of private houses and owners of apartments located in multi-apartment buildings of municipal ownership of the city should contact the district subscriber departments of the "Water Sales" Department at their place of residence (reception days: Monday-Thursday, 08:30-12:00, 13:00-16:00; Friday, Saturday – from 09:00 to 12:00):

• Subscriber Department of Khadzhibeysky District – Odessa, 126a Balkivska Street.
  
• Subscriber Department of Kyiv District – Odessa, 43a/2 Akademika Korolyova Street.
• Subscriber Department of Primorsky District – Odessa, 8 Pushkinska Street.
• Subscriber Department of Peresyp District – Odessa, 24 Heroiv Oborony Odesy Street.
  
• Private Sector Work Department (sections 1 and 2) – Odessa, 25 Mykhailivska Street.

For the period necessary for the consumer to implement agreed measures with the producer for the rational use of drinking water and the reduction of wastewater discharge and pollutants, the producer may establish temporary conditions for the acceptance of wastewater for the consumer, specified in the contract.

BUDGET

To conclude a contract for the provision of centralized water supply and sewage services, it is necessary to provide:

• technical conditions (also in case of increased water consumption, in other cases provided by law);
• agreed project and technical documentation;
• other necessary regulatory and technical acts;
• permission to connect to the network of the network owner;
• a document confirming the right of ownership (use) of the occupied premises;
• a copy of the order appointing responsible persons for water use and settlements for each object;
• location of objects;
• a detailed substantiated calculation of the water management balance for each object with the inclusion of the final data in the consolidated table;
• a water user questionnaire;
• an act of delimitation of responsibility between the Producer and the Consumer for water supply, sewage networks, and facilities;
• a list of third-party organizations, institutions, enterprises located on the territory of the object, indicating their details (postal, payment, full names of managers), water consumption and sewage volumes;
• a copy of the company`s charter (regulations for branches);
• a copy of the extract from the state register of legal entities and individual entrepreneurs;
• a copy of the certificate of inclusion in the Unified State Register of Enterprises and Organizations of Ukraine (EDRPOU) from the statistics department;
• a certificate of banking and postal details;
• a copy of the VAT payer certificate; Form 4-OPP certificate;
• copies of the order, meeting minutes on the appointment of the manager (if necessary, a power of attorney);
• a financing certificate; estimate.

OTHER (legal entities)

To conclude a contract for the provision of centralized water supply and sewage services, it is necessary to provide:

• technical conditions (in case of increased water consumption, in other cases provided by law);
• agreed project and technical documentation;
• other necessary regulatory and technical acts;
• permission to connect to the network of the network owner;
• a document confirming the right of ownership (use) of the occupied premises;
• a copy of the order appointing responsible persons for water use and settlements for each object;
• location of objects;
• a detailed substantiated calculation of the water management balance for each object with the inclusion of the final data in the consolidated table;
• a water user questionnaire;
• an act of delimitation of responsibility between the Producer and the Consumer for water supply, sewage networks, and facilities;

To conclude a contract for the provision of centralized water supply and sewage services, it is necessary to provide:

• technical conditions (also in case of increased water consumption, in other cases provided by law);
• agreed project and technical documentation;
• other necessary regulatory and technical acts;
• permission to connect to the network of the network owner;
• a document confirming the right of ownership (use) of the occupied premises;
• a copy of the order appointing responsible persons for water use and settlements for each object;
• location of objects;
• a detailed substantiated calculation of the water management balance for each object with the inclusion of the final data in the consolidated table;
• a water user questionnaire;
• an act of delimitation of responsibility between the Producer and the Consumer for water supply, sewage networks, and facilities;
• a list of third-party organizations, institutions, enterprises located on the territory of the object, indicating their details (postal, payment, full names of managers), water consumption and sewage volumes;
• a copy of the extract from the state register of legal entities and individual entrepreneurs;
• a copy of the passport;
• a copy of the tax identification number;
• a copy of the VAT payer or single tax certificate;
• Form 4-OPP certificate;
• a consent-notification for the processing of personal data;
• a consent to the collection and processing of personal data.

HOUSING COOPERATIVES, CONDOMINIUMS

To conclude a contract for the provision of centralized water supply and sewage services, it is necessary to provide:

• technical conditions;
• agreed project and technical documentation;
• other necessary regulatory and technical acts;
• permission to connect to the network of the network owner;
• title documents for the objects;
• documents confirming the balance sheet ownership of the object by the Consumer;
• a copy of the order appointing responsible persons for water use and settlements;
• information about residents who receive benefits for water use services with attached copies of relevant benefit documents;
• location of objects;
• a water user questionnaire;
• an act of delimitation of responsibility between the Producer and the Consumer for water supply, sewage networks, and facilities;
• a list of third-party organizations, institutions, enterprises located on the territory of the object, indicating their details (postal, payment, full names of managers), water consumption and sewage volumes;
• a copy of the charter of the housing cooperative (condominium);
• a copy of the extract from the state register of legal entities and individual entrepreneurs;
• a copy of the certificate of inclusion in the Unified State Register of Enterprises and Organizations of Ukraine (EDRPOU) from the statistics department;
• a certificate of banking and postal details;
• a copy of the VAT payer certificate; Form 4-OPP certificate;
• copies of the order, meeting minutes on the appointment of the manager (if necessary, a power of attorney);
• a financing certificate; estimate.
