Conclusion of a contract for centralized water supply and wastewater disposal services.

Contractual relations between producers and consumers:

Contractual relations regarding the use of centralized communal water supply and wastewater disposal systems are carried out exclusively on contractual grounds in accordance with the Laws of Ukraine "On Drinking Water and Drinking Water Supply", "On Housing and Communal Services", the Rules for using centralized communal water supply and wastewater disposal systems in settlements of Ukraine approved by the order of the Ministry of Regional Development, Construction, Housing and Communal Services of Ukraine dated June 27, 2008, No. 190, the "Rules for the Technical Operation of Water Supply and Wastewater Disposal Systems" approved by the order of the State Housing and Communal Services of Ukraine dated July 5, 1995, No. 30; "Rules for the acceptance of wastewater from enterprises, institutions, and organizations into the sewage system of Odessa", approved by the decision of the Executive Committee of the Odessa City Council dated June 14, 2007, No. 632; technical conditions, existing GOST standards, and other regulatory acts adopted in accordance with the law.

Essential terms of the contract between the producer and the consumer of centralized water supply and wastewater disposal services are determined in accordance with the Laws of Ukraine "On Drinking Water and Drinking Water Supply", "On Housing and Communal Services".

For the period necessary for the consumer to implement agreed measures with the producer to use drinking water rationally and reduce the volume of wastewater discharge and pollutants, the producer may establish temporary conditions for wastewater acceptance by the consumer as noted in the contract.

BUDGET

To conclude a contract for centralized water supply and wastewater disposal services, it is necessary to provide:

• technical conditions (also in case of increased water consumption volumes, in other cases provided by law);
• approved project and technical documentation;
• other necessary regulatory and technical acts;
• permission to connect to the network of the balance holder`s networks;
• a document confirming ownership rights (use) of the occupied premises;
• a copy of the order appointing responsible persons for water use and settlement for each object;
• location of objects;
• detailed reasoned calculation of the water balance for each object with the inclusion of summary data in the consolidated table;
• water user questionnaire;
• act of delineation of responsibility between the Producer and the Consumer for water supply, sewerage networks, and facilities;
• a list of third-party organizations, institutions, enterprises located on the object`s territory, indicating their details (postal, payment, full names of managers), water consumption-wastewater;
• a copy of the company`s charter (provisions, for branches);
• a copy of the extract from the State Register of Legal Entities and Individual Entrepreneurs;
• a certificate of inclusion in the Unified State Register of Enterprises and Organizations with a state management status;
• a certificate of banking and postal details;
• a copy of the VAT payer certificate; certificate – Form 4-OPP;
• copies of orders, meeting protocols on the appointment of a manager (if necessary, power of attorney);
• financial funding certificate; estimate.

OTHER (legal entities)

To conclude a contract for centralized water supply and wastewater disposal services, it is necessary to provide:

• technical conditions (in case of increased water consumption volumes, in other cases provided by law);
• approved project and technical documentation;
• other necessary regulatory and technical acts;
• permission to connect to the network of the balance holder`s networks;
• a document confirming ownership rights (use) of the occupied premises;
• a copy of the order appointing responsible persons for water use and settlement for each object;
• location of objects;
• detailed reasoned calculation of the water balance for each object with the inclusion of summary data in the consolidated table;
• water user questionnaire;
• act of delineation of responsibility between the Producer and the Consumer for water supply, sewerage networks, and facilities;
• a list of third-party organizations, institutions, enterprises located on the object`s territory, indicating their details (postal, payment, full names of managers), water consumption-wastewater;
• a copy of the company`s charter (provisions, for branches);

OTHER (individual entrepreneurs)

To conclude a contract for centralized water supply and wastewater disposal services, it is necessary to provide:

• technical conditions (also in case of increased water consumption volumes, in other cases provided by law);
• approved project and technical documentation;
• other necessary regulatory and technical acts;
• permission to connect to the network of the balance holder`s networks;
• a document confirming ownership rights (use) of the occupied premises;
• a copy of the order appointing responsible persons for water use and settlement for each object;
• location of objects;
• detailed reasoned calculation of the water balance for each object with the inclusion of summary data in the consolidated table;
• water user questionnaire;
• act of delineation of responsibility between the Producer and the Consumer for water supply, sewerage networks, and facilities;
• a list of third-party organizations, institutions, enterprises located on the object`s territory, indicating their details (postal, payment, full names of managers), water consumption-wastewater;
• a copy of the extract from the State Register of Legal Entities and Individual Entrepreneurs;
• a copy of the passport;
• a copy of the identification number assignment certificate;
• a copy of the VAT payer certificate or a single tax certificate; certificate – Form 4-OPP;
• consent-notification for personal data processing;
• consent for collection and processing of personal data.

Housing and Utility Cooperatives (HUC), HOAs

To conclude a contract for centralized water supply and wastewater disposal services, it is necessary to provide:

• technical conditions;
• approved project and technical documentation;
• other necessary regulatory and technical acts;
• permission to connect to the network of the balance holder`s networks;
• legal documents for objects;
• documents confirming the balance ownership of the object to the Consumer;
• a copy of the order appointing responsible persons for water use and settlement;
• information about residents who are entitled to benefits for payment of water use services with an appendix of copies of relevant privilege documents;
• location of objects;
• water user questionnaire;
• act of delineation of responsibility between the Producer and the Consumer regarding water supply, sewerage networks, and facilities;
• a list of third-party organizations, institutions, enterprises located on the object`s territory, indicating their details (postal, payment, full names of managers), water consumption-wastewater;
• a copy of the charter;
• a copy of the certificate of state registration of the enterprise;
• a copy of the inclusion in the Unified State Register of Enterprises and Organizations with a state management status;
• certificate of banking and postal details;
• a copy of the VAT payer certificate or non-profit status certificate, certificate – Form 4-OPP;
• documents confirming the position and powers of the manager signing the contract (protocol, appointment order, contract, other documents).
