Conclusion of contracts for the population

Contractual Relations between Producers and Consumers:

Contractual relations regarding the use of centralized municipal water supply and sewerage systems are carried out exclusively on a contractual basis in accordance with the Laws of Ukraine "On Drinking Water and Drinking Water Supply", "On Housing and Communal Services", the Rules for Using Centralized Municipal Water Supply and Sewerage Systems in Settlements of Ukraine, approved by the order of the Ministry of Housing and Communal Services of Ukraine dated 27.06.2008 No. 190, the "Rules for the Technical Operation of Water Supply and Sewerage Systems", approved by the order of the State Housing and Communal Services of Ukraine dated 05.07.1995 No. 30; "Rules for the Acceptance of Wastewater from Enterprises, Institutions, and Organizations into the Sewerage System of Odesa", approved by the decision of the Executive Committee of the Odesa City Council dated 14.06.2007 No. 632; technical conditions, existing GOST standards, and other regulatory acts adopted in accordance with the law.

Essential terms of the contract between the producer and the consumer of centralized water supply and sewerage services are determined in accordance with the Laws of Ukraine "On Drinking Water and Drinking Water Supply", "On Housing and Communal Services".

For the conclusion of contracts for the provision of centralized water supply and sewerage services, legal entities and individual entrepreneurs should contact the Control and Industrial Water Use Department of the Infoksvodokanal branch at the address: Odesa, 43a Academician Korolyov Street, contact phone 705-55-21.

For the conclusion of contracts for private house owners, apartment owners in municipal multi-apartment buildings, you should contact the district subscriber departments of the "Vodozbut" Management at the place of residence (reception days: Monday-Thursday, 08:30-12:00, 13:00-16:00; Friday, Saturday - from 09:00 to 12:00):

For the period required by the consumer to implement measures agreed with the producer for the rational use of drinking water and the reduction of wastewater discharge and pollutants, the producer may set temporary conditions for the acceptance of wastewater, indicated in the contract.

BUDGET

To conclude a contract for the provision of centralized water supply and sewerage services, the following documents are required:

• technical conditions (also in case of increased water consumption, in other cases provided by law);
• agreed project and technical documentation;
• other necessary regulatory and technical acts;
• permission to connect to the network of the network balance holder;
• document confirming the right of ownership (use) of the occupied premises;
• copy of the order appointing persons responsible for water use and accounting for each object;
• dislocation of objects;
• detailed justified calculation of the water management balance for each object with the final data entered into a summary table;
• water user questionnaire;
• act of delimitation of responsibility between the Producer and the Consumer for water supply, sewerage networks, and facilities;
• list of external organizations, institutions, enterprises located on the territory of the object, indicating their details (postal, payment, name of managers), volumes of water consumption and sewerage;
• copy of the company`s charter (regulations for branches);
• copy of the extract from the state register of legal entities and individual entrepreneurs;
• copy of the certificate of inclusion in the EDRPOU from the statistics office;
• certificate of banking and postal details;
• copy of the VAT payer`s certificate; certificate - Form 4-OPP;
• copies of the order, minutes of the meeting appointing the head (if necessary, power of attorney);
• funding certificate; estimate.

OTHER (legal entities)

To conclude a contract for the provision of centralized water supply and sewerage services, the following documents are required:

• technical conditions (in case of increased water consumption, in other cases provided by law);
• agreed project and technical documentation;
• other necessary regulatory and technical acts;
• permission to connect to the network of the network balance holder;
• document confirming the right of ownership (use) of the occupied premises;
• copy of the order appointing persons responsible for water use and accounting for each object;
• dislocation of objects;
• detailed justified calculation of the water management balance for each object with the final data entered into a summary table;
• water user questionnaire;
• act of delimitation of responsibility between the Producer and the Consumer for water supply, sewerage networks, and facilities;
• list of external organizations, institutions, enterprises located on the territory of the object, indicating their details (postal, payment, name of managers), volumes of water consumption and sewerage;

• other necessary regulatory and technical acts;
• permission to connect to the network of the network balance holder;
• document confirming the right of ownership (use) of the occupied premises;
• copy of the order appointing persons responsible for water use and accounting for each object;
• dislocation of objects;
• detailed justified calculation of the water management balance for each object with the final data entered into a summary table;
• water user questionnaire;
• act of delimitation of responsibility between the Producer and the Consumer for water supply, sewerage networks, and facilities;
• list of external organizations, institutions, enterprises located on the territory of the object, indicating their details (postal, payment, name of managers), volumes of water consumption and sewerage;
• copy of the extract from the state register of legal entities and individual entrepreneurs;
• copy of the passport;
• copy of the certificate of assignment of identification number;
• copy of the VAT payer`s certificate or single tax certificate;
• certificate - Form 4-OPP;
• consent-notification for the processing of personal data;
• consent for the collection and processing of personal data.

HOAs and Co-operatives

To conclude a contract for the provision of centralized water supply and sewerage services, the following documents are required:

• technical conditions;
• agreed project and technical documentation;
• other necessary regulatory and technical acts;
• permission to connect to the network of the network balance holder;
• ownership documents for the objects;
• documents confirming the balance ownership of the object by the Consumer;
• copy of the order appointing persons responsible for water use

• Information about residents who receive benefits for water service payments with attached copies of relevant preferential documents;

• Location of facilities;

• Water user questionnaire;

• Responsibility division act between the Producer and the Consumer regarding water supply, sewerage networks, and facilities;

• List of third-party organizations, institutions, enterprises located on the facility`s territory, specifying their details (postal, payment, full names of directors), volumes of water consumption and wastewater discharge;

• Copy of the statute;

• Copy of the certificate of state registration of the enterprise;

• Copy of the extract from the Unified State Register of Enterprises and Organizations of Ukraine (EDRPOU) with the status information;

• Statement of bank and postal details;

• Copy of the VAT payer certificate or proof of non-profit status, Form 4-OPP certificate;

• Documents confirming the position and authority of the signatory manager (protocol, appointment order, contract and other documents).

POPULATION (owners of private houses, owners of apartments located in multi-apartment buildings of municipal property of the city)

TYPICAL INDIVIDUAL AGREEMENT for provision of centralized water supply and centralized wastewater services;

List of addresses of multi-apartment buildings for which the model of individual public contract is determined

To conclude an agreement for the provision of centralized water supply and wastewater services, it is necessary to provide:

• Copy of passport;
• Copy of the identification number assignment certificate;
• Document confirming the right of ownership (use) of the real estate object;
• Document confirming the consumer`s benefits;
• Family composition certificate Form;
• Meter passport installed in the apartment.
